Seguin 2026-2027 - Letter of Intent (Sports, Signature Experiences, Clubs, and/or Events)

Parent Consent for Student Participation in Sports, Signature Experiences, Clubs & School Events

Southwest Preparatory School is excited to offer students opportunities beyond the classroom through clubs, events, and signature experiences that enrich learning and build community. To ensure that all students can participate safely and successfully, we require parent consent for their child’s involvement. This form must be signed by a parent/guardian and will remain on file for the remainder of the school year.

 

Please review the expectations below, as students must meet the listed requirements to remain eligible for participation in sports, clubs, field trips, and other school-sponsored activities.

 

All eligible Southwest Preparatory Seguin students have the opportunity to attend upcoming sports, clubs, events, and signature experiences. We have many wonderful things planned for the remainder of the school year, and much more.

 

In order to be eligible for participation in any sports, clubs or upcoming events and field trips, you will be expected to:

 

  • Maintain a grade of 70% and above in all core classes (Reading, English Language Arts, Math, Science and Social Studies).

  • Maintain good attendance. No more than 5 unexcused absences during a 9-week grading period meets the general eligibility requirements. 

  • Demonstrate good behavior daily. No detentions, write-ups, or suspensions will meet the general eligibility requirements.

 

Students must meet these requirements monthly for clubs and up to the date of any start of the sport, event or field trip. The Principal of SWREP- Seguin reserves the right to exclude a student from participating in the school activity if they fail to meet the qualifications listed above or whose behavior may pose a disciplinary or safety risk at the field trip or event.

  5. Flag Football 7th-HS
  6. Volleyball 5th-8th
  7. Basketball
  8. Soccer 3rd- HS
  9. Dungeon & Dragons 5th-12th
  10. Lego PK-1st
  11. Coloring PK-2nd
  12. Environmental Club 5th-12th
  13. Book Club 8th-12th

As resident of the city of Seguin, County of Guadalupe, State of Texas, I do hereby authorize my child to participate in this field trip. Transportation will be provided by SWPS using our school transportation.*

I acknowledge that my child has no medical limitations and is fully capable of participating in said activities. I appoint SWPS to act on my behalf in the event that my child should require emergency medical attention while participating in this field trip. This appointment gives SWPS the authority to sign releases to physicians who may render medical care if it becomes necessary in case of an emergency. I agree to assume liability for payment of all professional services and reimburse SWPS for any expense that it may incur resulting from any medical services for my child. I hereby agree to hold Southwest Preparatory School District, SWPS employees and any other agent of SWPS who may act on my behalf, harmless in any decision regarding the care and treatment of my child in the event of injury or other emergency situation.*
