Required Documentation for Enrollment

The following documents are required for enrollment at Southwest Preparatory School

• Proof of Residency (Utility Bill, Telephone, or Property Tax Bills; rental lease agreement)
• Copy of Social Security Card (This disclosure is voluntary and the SS# is used for student ID in PEIMS) 
• Copy of proof of identity (birth record, driver's license, passport, military id, transcripts, report card, adoption record) 
• ​Transcripts (official or unofficial), report card, testing records, CSR, State Assessment scores, PGP, Career Pathways Portfolio, & Special Education records if applicable 
• Shot Records indicating current immunizations 
• ​Proof of Guardianship, if applicable
